How shall the personnel subject to charge-free technical service on basic Items of family planning conduct reimburse procedures?

The applier shall fill in "Verification Form for Expense Reimbursement of Technical Service on Basic Items of Family Planning" and provide the original copy of related evidence documents at the request of regulations when he conducts reimbursement procedure:

1) Who is married and is with the registered permanent residence of Shanghai, shall provide his own household register, marriage certificate, and the specific receipt of clinic /emergent treatment (hospitalization) expenditure and the charge list provided by the service institution selected by the district (county) government or town (village) government in charge of his registered permanent residence;

2) Who is married and is with the registered permanent residence of other province or municipality, and has participated in Shanghai Comprehensive Insurance Program for Immigrant Employee, shall provide his own ID card, marriage certificate, "Marriage and Child-bearing Certificate of Immigrant Employee", "Shanghai Temporary Residence Certificate of Immigrant Population", the evidence of participating in Shanghai Comprehensive Insurance Program for Immigrant Employee, and the specific receipt of clinic /emergent treatment (hospitalization) expenditure and the charge list provided by the service institution selected by the district (county) government or town (village) government of where he lives at present;

3) Who is married and has got "Shanghai Residence Certificate", shall provide his own ID card (not applicable to the cross-national talent with "Shanghai Residence Certificate" V.B), "Shanghai Residence Certificate", marriage certificate, "Marriage and Child-bearing Certificate of Immigrant Employee" (not applicable to the cross-national talent with "Shanghai Residence Certificate" V.B), and the specific receipt of clinic /emergent treatment (hospitalization) expenditure and the charge list provided by the service institution selected by the district (county) government or town (village) government of where he lives at present;

4) Who is with the registered permanent residence of other province or municipality, and has married someone with the registered permanent residence of Shanghai, shall provide his own ID card, marriage certificate, "Marriage and Child-bearing Certificate of Immigrant Employee", "Shanghai Temporary Residence Certificate of Immigrant Population", and the specific receipt of clinic /emergent treatment (hospitalization) expenditure and the charge list provided by the service institution selected by the district (county) government or town (village) government of where he lives at present. If the applier entrusts someone else to handle reimbursement procedures, the entrusted part shall provide the original copy of his own ID card as well.
