Holding valid Qualification of Commercial Road Transportation Drivers
(I) Renewal of Qualification of Commercial Road Transportation Drivers on
expiration:
1. Original copies and photocopies of the applicant¡¯s driver license and ID;
2. Original copy of the applicant¡¯s Occupational Qualification;
3. 2 copies of 1¡±colored digital photos of the applicant;
4. Renewal / Reissue Registration Form of Road Transport Occupational
Qualification (refer to Annex VII).
(II) Reissue of Lost Qualification of Commercial Road Transportation Drivers:
1. Original copies and photocopies of the applicant¡¯s driver license and ID;
2. 2 copies of 1¡±colored digital photos of the applicant;
3. Renewal / Reissue Registration Form of Road Transport Occupational
Qualification (refer to Annex VII);
4. 1 copy of the Certificate of Loss Declaration of Qualification of
Commercial Road Transportation Drivers.
(III) Transfer-in of Qualification of Commercial Road Transportation Drivers:
1. Archive about the Qualification of Commercial Road Transportation Drivers
given by the qualification issuing administration of the transfer-out location.
2. Original copies and photocopies of the applicant¡¯s driver license and ID;
3. 2 copies of 1¡±colored digital photos of the applicant;
4. Registration Application Form of Qualification of Commercial Road
Transportation Drivers (refer to annex V). </P>
1. The applicant submit the materials to the Reception Center,
2. Application with valid and complete materials will be finished on spot by
the Reception Center according to the working requirements,
3. The Reception Center shall deliver the materials to the municipal
transport administration with 10 days as of the completion.
