a. With disability in lower limbs, holding the Certificate of Disability verified by the Disabled Persons’Federation or Certificate of Disability in Lower Limbs issued by Disabled Persons’Federation of the district or county;

b. Aged between 18 and 65, and with permanent residence registration in Shanghai;

c. Aged above 14 for application of manual disability-adapted vehicle;

d. Demobilized, retired or transferred soldiers with disability in lower limbs:

·Certificate of Disability issued by the troop;

·permanent residence registration in Shanghai;

·Certificate of Disability in Lower Limbs issued by Disabled Persons’Federation of the district or county;

·refer to paragraph b and c for other requirements;

e. Persons coming to Shanghai for provisional medical treatment with disability in lower limbs:

·by the ID, Certificate of Disability or Certificate of Disability in Lower Limbs;

·Temporary Residence Permit issued by public security agencies of Shanghai;

·Certificate of physical re-examination in designated hospitals of Shanghai;

·Get the Application Form of License Plate of Disability-Adapted Vehicle Registration from the Non-motor Vehicle Administration of the residence location;

·refer to paragraph b and c for other requirements for provisional license plate of disability-adapted vehicle.

a. ID of Shanghai (or residence registration) and Certificate of Disability;

b. Receipt of vehicle purchase and certificate of qualification;

c. 4 1”recent bareheaded photos;

d. Should the receipt of vehicle purchase and certificate of qualification be lost, the vehicle manufacturer or dealer shall re-issue the receipt or valid certificates;

e. Insurance Certificate of Third-party Liability (except for manual disability-adapted vehicles);

f. The disability-adapted vehicle shall comply with GB12995-91 National Standard of Motor Wheelchairs and Provisional Supplementary Regulations on the Implementation of GB12995-91 National Standard (Motor Wheelchairs) of Shanghai, and be listed in the“Product list of disability-adapted vehicles applicable to license plate in Shanghai”promulgated by Shanghai Municipal Public Security Bureau Headquarter of Traffic and Patrol Police in 1999, including new manual and battery disability-adapted vehicle;

And manufacturers and part numbers verified by Shanghai Economic & Trade Commission in Technical Evaluation of New Products in 2001.

g. For both persons with disabilities in lower limbs of Shanghai or provisional stay in Shanghai for treatment, application for 2 disability-adapted vehicle license plate (including provisional number plate) by 1 person is forbidden. The valid period of provisional number plates shall be 6 months.

a. Verify the vouchers and certificates;

b. Examine the size of the vehicle, engine displacement and safety facilities;

c. Fill the Registration Form of License Plate Application for Disability-Adapted Vehicle of Shanghai;

d. Stamp the steel seal of vehicle public security;

e. Fill the vehicle license and files with signatures and seals;
