Application of Shanghai Practicing Pharmacist Registration

Shanghai Food and Drug Administration

Application scope:

Person obtaining practicing pharmacist qualification, working in drug production, operation, use unit and conforming to registration conditions.

Application qualification/condition:

£¨I£©Those applying for practicing pharmacist must conform to the following conditions simultaneously:

1£®Obtain Practicing pharmacist qualification certificate;

2£®Complete the credits of further education for practicing pharmacist in line with requirements;

3£®Abide by discipline and law, conform to pharmacist ethics;

4£®Good health, competent of pharmacist position work;

5£®Recommendation by the practicing unit with employment relationships

£¨II£©No registration for people with one of the following conditions:

1£®Without full civil capacity;

2£®Receive criminal punishment, no more than two years from the completion of the punishment to the date of registration;

3£®Receive punishment of canceling the registration of practicing pharmacist qualification no more than 2 years;

4£®Other situations which the state regulates as not appropriate to practice pharmacist.

£¨I£©Registration application

1£®Person who first registers must fill in¡°Practicing Pharmacist First Registration Application Form¡±, and submit related documents see appendix.

2£®The valid period of practicing pharmacist registration is three years. The certificate bearer must apply for a second time registration formality in the original registration institute one month prior to the expiration of the registration, fill in practicing pharmacist second time registration application form, and submit related documents (see appendix) with the original, duplicate copy of practicing pharmacist registration certificate.

3£®If changing practicing unit or scope in the same practicing area, the practicing pharmacist must handle registration modification formalities in the original registration institute, and fill in fill in practicing pharmacist registration modification form, and submit related documents (see appendix) with the original, duplicate copy of practicing pharmacist registration certificate. If changing practicing area, the practicing pharmacist must handle registration modification formalities in the original registration institute, and fill in fill in practicing pharmacist registration modification form, and apply for registration again in the practicing pharmacist registration institute in the new area, and fill in¡°Practicing Pharmacist First Registration Application Form¡±, and submit related documents (see appendix) with the original, duplicate copy of practicing pharmacist registration certificate.

4£®Apply for cancelling registration: if with one of he following situations, practicing pharmacist shall be canceled for registration:

£¨1£©Die or declared to be lost;

£¨2£©Receive criminal punishment;

£¨3£©Practicing pharmacist certificate was revoked;

£¨4£©Receive administrative punishment of dismissal;

£¨5£©Not able to conduct pharmacist business due to health or other reasons.

The registration cancel formalities shall be applied for acceptance to the registration institution by the unit where the practicing pharmacist works within 30 working days, and must fill in practicing pharmacist registration cancel form, and submit related documents (see appendix) and the original, duplicate copy of practicing pharmacist registration certificate.

£¨II£©Acceptance£º

The practicing pharmacist registration work shall be completed within 20 days after acceptance.

The office of Shanghai Municipal Practicing Pharmacist Work Committee will finish the preliminary examination of the application documents within 4 working days after receiving the application documents, and notify the applicant if supplement is needed. The applicant shall make up the documents within required time limit, otherwise the examination terminates. The reexamination will be completed within 6 working days and propose the suggestions on registration or not. Special cases and the non-registration material shall be submitted to the personnel education division of Shanghai Food and Drug Administration to decide whether register or not. For those with approved registration, the certificate shall be made within 10 working days. For those with no registration, the applicant shall be notified in written form.
