The examination decision shall be made within 20 working days after the
health authorities accept the application
1. Continuous registration of nurses in the medical institutes within the
municipality;
2. Other nursing personnel employed in nursing position of other institutes
regulated by the municipal health administrative department.
Note: please apply for continuous registration 60 days prior to the
expiration of the registration.
1. Nurse second time registration application form;
2. Statement of the situation where the individual can not participate in
collective continuous registration (public seal of the unit needed);
3. For registration suspension more than 5 years, the applicant needs to
submit the original¡°Shanghai Municipal In-Service Education Registration Book
(Nursing)¡±(time requirement for in-service education:¡Ý15 hours. Academic timing
credential shall conform to the registration of in-service education. The number
of academic hours shall be calculated, confirmed, stamped by the nursing
department of the employer hospital.)
4. For registration suspension more than 5 years, the applicant need to
participate in nursing position for clinical practice in comprehensive hospital
above A level of Grade II, and submit the clinical practice credential issued by
the medical institute;
5. The original nurse Practicing certificate (minor certificate)
