The examination decision shall be made within 20 working days after the
health authorities accept the application
Having obtained nurse practicing qualification (e.g. having obtained the
major certificate), employed or intended to be employed in the following
positions:
1. Nursing personnel in medical institutes;
2. Teaching staff of nursing major in medical school, health school, and
medical institutes;
3. Professionals for nursing management in health bureau, nursing
association, advanced medical school, medical institutes.
1. Nurses¡¯first registration application form (Individual application shall
fill in an additional Shanghai nurse first registration application form)
2. Nurse Practicing Certificate of People¡¯s Republic of China (Major
certificate)
3. Replicate copy of identity credentials
4. Unit intended employment credential (no such credential needed for current
employees, yet the employer unit shall note in the application form and
application sheet of being in service);
5. Applicant who intends to practice in private or internal medical institute
shall provide the duplicate of the institute¡¯s Medical Institute Operation
License;
6. Nurse registration health check form (issued by hospital above Grade II);
7. 3 recent small 2-inch front photos (without hat) (each of the two photos
to be stuck in Nurse first registration application form and Nurse registration
health form by the applicant);
8. Applicant who has not obtained the minor certificate five years after
obtaining nurse practicing qualification needs to participate in nursing
position for clinical practice in comprehensive hospital above A level of Grade
II, and submit the working credential issued by the medical institute; or submit
working credential of nursing position in recent 5 years.
Note: for applicants who obtained the Nurse Practicing Certificate of
People¡¯s Republic of China (Major certificate), and the code of the nurse
practicing certificate starts with¡°4¡±, they need to further provide
non-registration credential, title certificate and graduate diploma.
