The examination decision shall be made within 20 working days after the health authorities accept the application

Practicing personnel in medical institute holding Practicing certificate in medical institute issues by Shanghai Municipal Health Bureau

（A4 size paper replicate copy）

1. Doctor practicing registration application examination form;

2. 3 recent small 2-inch front photos (without hat, same film) (each of two in application form and body check form);

3. Duplicate of the applicant’s ID card;

4. The original and duplicate of doctor qualification certificate;

5. The applicant’s body check form within 6 months (issued by medical institute above Grade II);

6. Person who have obtained the practicing (assistant) doctor qualification with no registration within 2 years or suspended doctor practicing more than 2 years applies for registration, the applicant shall submit additional certificate of receiving 3-6 months’training in medical, prevention, health care institutes and qualified in the evaluation—“Training Certificate of Practicing (Assistant) Doctor in Shanghai”;

a）Doctors working in Grade III medical, prevention, health care institutes shall be trained by the original institute and issue training certificate;

b）Doctors working in other medical, prevention, health care institutes shall be trained by the training institute designated by district (county) health administrative department and show training certificate.

7. The replicate copy of the medical institute operation certificate (including medical institute name, institute registration number, address, postal code, diagnosis subject)

8. Employment credentials by medical, prevention, health care institutes (for currently in-position personnel, the practicing institute fill in the“practicing institute’s opinion”column of the Medical practitioner registration application examination form and stamp a seal).
