Where do I apply to establish a joint venture/cooperative medical institution? What documents need to be submitted when applying?

Applications may be submitted to the public health administration of the district/county where the joint venture/cooperative medical institution is to be located.
You need to submit:
1. Application to establish a joint venture/cooperative medical institution.
2. Project plan signed by both parties.
3. Feasibility report for the establishment.
4. Certificates of medical institution registration of both parties.
5. Personal identification of the legal representative of the joint venture/cooperative (copy).
6. Bank credit certificate
7. Government approval or evaluation of State assets invested in the joint venture/cooperative medical institution.
