Personnel going to Hong Kong or Macao for training program.

1. To complete and submit the Form of Examination and Approval on Chinese Citizens Applying for going to and fro Hong Kong or Macao pasted with a full-faced colored picture which is taken recently (48X33mm, with white color as the background), attached with another picture of the same dimension.

2. Applicants shall present for examination the originals of valid Resident Identification Cards, and residential registration booklets (applicants from other places in China but outside Shanghai shall present for examination their temporary residence permit or Blue seal residential registration booklet), and submit their photocopies (those filing in the application for the second time are exempted from the photocopies).

Overseas Chinese shall present for examination their valid Chinese passports and certifications on their residing abroad, and submit photocopies of the documents

3. To present for examination the originals of Entry Permit for training program, and submit photocopy of the Permit.

4. To submit written opinion from the work unit of the applicant or from public security organs.

The written opinions can be filled in the corresponding column of Form of Examination and Approval on Chinese Citizens Applying for going to and fro Hong Kong or Macao, and shall be stamped with official seal, or can be issued separately.

5. Other materials assumed to be submitted by entry-exit administration organs.

Where applicants file in the application for a second time within the term of validity of Exit-entry Permit for Traveling to and from Hong Kong and Macao, written opinions from their work unit or form public security organs are exempted. Where permit holders lost their Exit-entry Permit for Traveling to and from Hong Kong and Macao, they shall submit a declaration on the loss case and nullifying the lost Permit published on newspaper on municipality level (i.e, Jiefang Daily, Weihuibao and Xinmin Evening Post).

Where it is the application for the first time, the applicant shall file in the application in person. If it is the application for the second time, the applicant can entrust somebody to go for the application, and the factor shall present for examination his own identity certification and the identity certification of the applicant.
