Anyone who loses his Exit-entry Permit for Traveling to and from Hong Kong
and Macao
1. To complete and submit the Application Form for Shanghai Residents
Traveling to and from Hong Kong and Macao pasted with a 2¡¯¡¯barehead front-view
colored picture which is taken recently (48X33mm, with white color as the
background), attached with two other pictures of the same dimension;
2. To present, for examination originals of valid ID card, residential
registration booklet or other documents on the registered residence, attached
with their photocopies.
Overseas Chinese filing in the application shall present for examination the
valid passport and documents certifying his residing overseas, attached with
their photocopies.
3. To submit a declaration on the loss case and nullifying the lost
Exit-entry Permit for Traveling to and from Hong Kong and Macao published on
newspaper on municipality level (i.e, Jiefang Daily, Weihuibao and Xinmin
Evening Post).
4. If application for an endorsement on the lost permit is also made,
relevant documents are to be provided.
The applicant must go to file in the application in person.

Examination and Approval and Issuance of Application for Exit-entry Permit for
Traveling to and from Hong Kong and Macao.
100RMB for an Exit-entry Permit for Traveling to and from Hong Kong and
Macao; 20RMB for a single-journey endorsement; 100RMB for a multiple-journey
endorsement valid within one year, and 300RMB for a multiple-journey endorsement
valid for three years.
