Applicants must go for the application procedures in person. Those younger than 16 or older than 70 can entrust somebody for the application (the factor must present his Resident Identification Card).

Where the passport is within its term of validity and comes under one of the following circumstances, and if the passport is damaged, the change of passport can be done after 3 months from the day when the application is accepted:

1. The attached pages for endorsement have all been used up; 

2. Quality of the passport is not up to the standard;

3. Serial number of the Resident Identification Card, birthplace or name of the passport holder changes; 

Notice: application for extension endorsement on the 1992 version passport is to be changed to 1997 version passport, and an endorsement on changing passport shall be made.

1. To submit a completed Passport Application Form pasted with a full-faced color photo of the applicant which is taken recently (with a dimension of being 48X33mm, and white color as its background), attached with two other photos of the same dimension.

2. To present and submit residential registration booklet for Shanghai resident (or the original of certification on registered residence) of the applicant, and the photocopies of his ID card, presenting the original of the card for examination at the same time. 

3. To submit the photocopies of the information pages and pages for endorsement of the original passport, and present for examination the originals of the pages. 

4. Where application for changing passport on account of the changes of contents in passport occur, the applicant shall present corresponding certifications on residence registration.

5. Where the passport is damaged, the change of passport shall be done after three months from the day when the application is accepted.

6. Where the passport is altered, and the violator has accepted punishment from relevant department, the application for changing a passport shall be accepted in 3 months after the punishment is enforced.  

7. Where the passport holder does not have a permanent registered residence in Shanghai, he shall present and submit certifications on his residing overseas (i.e., green card or settlement visa) and Certificate on Temporary Residence for Residents of Hong Kong and Macao and for Overseas Chinese (or certification on cancellation of residence registration of the passport holder). 

8. Where other information is endorsed, corresponding evidence shall be provided.

9. The following persons (with special identity) shall provide a written opinion of their work units:  

（1）Officials in the municipal department who are incumbent or retired (with honor);

（2）Officers on department level in all organs including people’s organizations of worker, youth and women, as well as those from democratic parties;

（3）Those working on the key posts at enterprises and public institutions related to the state politics, economy, security and commercial secrete, and finance, taxation, scientific research and education, etc;

（4）Those working at Shanghai-based units under direct control of the central government, or those at units of other provinces, municipalities or autonomous regions stationed in Shanghai.

Note: applicant must go in person for the application procedures.
