1. Registered permanent residence certificate and ID cards of the parents of the child to be notarized;
2. Birth testimonial, to be completed by the mother or father, or someone who is familiar with the birth situation;
3. Documents evidencing the relationship between the parents and the child (not necessary if the registered permanent residence certificate is provided);
4. Birth certificate or the only-child certificate.
