To apply to reissue a passport

If the original passport of the passport holder is lost within its valid period, the passport holder can apply to be reissued a new one. If the original passport expired, the application to reissue a new one shall not be accepted.

If the passport is lost before the passport holder leaves the country and application is made to reissue a new one, a new passport can be reissued after 3 months from the day when the loss is reported.

If a passport is lost when the passport holder comes back to the country on short term, Entry-Exit Administration Bureau of Shanghai is to verify the situation with the department issuing the passport or with frontier inspection offices at entry-exit port before deciding whether to reissue a new passport or not according to the specific circumstance.

1. To complete and submit an Application Form for Changes with Passport pasted with a 2¡¯¡¯full-faced colored picture for the use of certificate on mirror side photographic paper(with a dimension of being 48¡Á33mm, and white color as its background), attached with three other pictures of the same dimension;

2. To submit document proving the loss case issued by public security department of the place where the passport is lost (or of the place where the applicant has his (or her) registered permanent residence).

3. To submit a declaration on the loss case and nullifying the lost passport published on newspaper on municipality level (i.e, Jiefang Daily, Weihuibao and Xinmin Evening Post).

4. If there is information on any endorsement on the lost passport, relevant documents are to be provided. The applicant must go to file in the application in person.

For ordinary passport, it is 200RMB per passport; it is 400RMB per passport for a reissued one; it is 20RMB for an endorsement or extension on period of validity.
