The dead person’s family members or reception unit in Shanghai, officials from the consulate of the dead person’s country.

1. The dead person’s passport or certificate. The residence permit shall be handed in and canceled if there is one;

2. Death testimony materials:

a. Stamped death certificate issued by hospitals above county level;

b. For abnormal death, the documentary evidence of death report issued by public security department shall be provided;

c. For death in a district, death certificate issued by community clinical center or township clinic is need, or relevant certificate issued by police office, police station;

3. The formalities of a foreigner’s death should be notified by his or her family members, and notification by non-relatives shall provide the commission letter approved by the General Consulate in Shanghai or Embassy in China.
