1. Acceptance
When the application is accepted, the applicant fills in the application
form.
2. Examination
Within five days after the valid application form is accepted, all
application documents will be examined. In case of submission of incomplete or
unqualified documents, the applicant will be informed to replenish the required
documents. The applicant who fails to replenish the documents within one month
will have to reapply for the certificate. Upon completion of all required
documents replenished, the examination will be conducted within 10 days. The
examination result will come out and reach to the applicant within five days,
with the applicant being granted with the Animal Epidemic-prevented Certificate
or being officially replied.
3. Second examination
The applicant who fails in the initial examination can apply for a second
examination after he/she makes modification according to the requirement. The
application for second examination will be examined within 10 days after the
documents are accepted. The examination result will come out and reach to the
applicant within 10 days, with the applicant being granted with the Animal
Epidemic-prevented Certificate or being officially replied in written.
Organizations or individuals who are engaged in breeding, butchery,
processing, transport, sales and medical treatment on animals and processing,
sales and reservation of animal products in Shanghai.
1. Written application (print copy);
2. Ownership or lease certification of the operation place(s) (original and
photocopy);
3. ID card of legal person and the employment certification (original and
photocopy);
4. Business license and sanitation permit (original and photocopy);
5. Training certificate of veterinary sanitation specialist.
Examining Standard of Shanghai on Animal Epidemic Prevention, No. 34 of
HUXUMUBAN (2003), applies.
