evaluation report;
5. Construction layout plan of the medical
institution;
6. Rules and regulations of the medical institution;
7.
Certification document of incumbency, qualification certification and
professional certificate of the legal representative or main principal of the
medical institution;
8. Name list of the principal of all departments of the
medical institution;
9. Approval document of the completion of construction
for the medical institution newly built, rebuilt or rebuilt at a new
place;
10. Contract or agreement for the medical institution established upon
joint investment;
11. Other materials required by the authority for public
hygiene.
Note: If the application for establishment and for registration of
operation is submitted to municipal authority for public hygiene, the applicant
shall submit the written comment on the preliminary review issued by the
district/county authority for public hygiene in charge of the medical
institution

qualification: After obtaining the "Approval of Establishment of Medical
Institution", the medical institution shall apply to the authority for public
hygiene that approves its establishment and issues the "Approval of
Establishment of Medical Institution" for registration of operation when it
plans to begin operation within the period of validity and shall begin the
operation after obtaining the "License of Operation of Medical Institution".
