Application Formalities:
(I) presenting the Funeral
Service Card;
(II) presenting the annual financial statement; and
(III)
presenting the Report of Annual Review of Funeral Parlours in
Shanghai.
Processing Procedures:
Any and all of funeral parlours shall,
within ten days upon receipt of the annual review notice, present any and all of
materials necessary for annual review to the funeral administration department
at district/county level which shall, upon first review and approval, report to
Shanghai Funeral Administration for final review and approval.

Time Limit for Processing: The funeral
department at district/county level shall, within fifteen days upon receipt of
the Report of Annual Review of Funeral Parlours in Shanghai, make the decision
upon first review. Shanghai Funeral Administration shall, within fifteen days
upon receipt of the approval decision upon first review by the funeral
department at district/county level, make the approval decision upon final
review or not.
