Application Formalities:
Processing Procedures:
(I) The taxpayer shall apply in writing to the
competent taxation authority for print of invoices.
(II) The taxpayer shall,
upon the Printed Book for Purchase of Invoices and the supporting document for
the personal identity of the person in charge of application, get and fill in
the Application Form for Print of the Unified Invoices of Shanghai Municipality
attached with two sets of specimens of invoices ready for print covering the
following items:
1. the business name, type and size of text, number and
purpose of duplicate, name of client, bank of deposit, and number of account;
and
2. name of commodity or current account, unit of measurement, quantity,
unit price, separate and gross amounts, name of drawer, date of drawing, seal of
drawing unit (or individual).
(III) The competent taxation authority shall
review the aforesaid materials presented by the taxpayer.

Processing Time: 1. On Mondays
through Fridays except for statutory festivals or holidays.

Time Limit for Processing: The competent taxation authority shall,
within seven working days upon receipt of the Printed Book for Purchase of
Invoices, the supporting document for the personal identity of the person in
charge of application, the Application Form for Print of the Unified Invoices of
Shanghai Municipality and the required specimens of invoices ready for print,
complete review whereof.
Qualified Application Subject: Any and all of
taxpayers with the legitimate and valid the Printed Books for Purchase of
Invoices and supporting documents for the personal identities of persons in
charge of purchase of invoices.
