5. Identification, education, registered veterinarian qualification, vocational permit and relationship documents of veterinarian recruited (original and copy)
6. List of equipments in the clinic
7. Proof of waste treatment (original and copy)

Handling Procedure:
1. Location Selection
The clinic shall meet the requirement of animal epidemic prevention and public health. Shall be far away from school, hospital, restaurant, cinema, stadium and other crowded areas.
2. Layout
The actual floor area of the clinic shall be more than 100 square meters. There should be a specific office, treatment room, operation room, medicine room, laboratory and separated room for diseased animals.
3. Staff
（1）There should be more than two registered veterinarians (with vocational qualification) and other assistants to carry out necessary out. These staff shall receive regular training, and shall be provided with health and job insurance as well as retirement pension. (2) All staff working in the clinic shall have the health certificate issued by the relevant departments of public health.
4. Management System

（2）Hygiene and Epidemic Prevention System
（3）Staff accountability system
（4）Charging standards
（5）Supervising and complaining room
5. Clinic equipments
Necessary animal treatment equipment for treatment, operation, sterilization, freezing and common assessment.
6. Keeping File
Files shall be kept on medicines that enter or exit warehouse, on the use of medicines, on sterilization, disease treatment, prescription, dead animals and used equipments.
7. Use of Veterinary Medicines
Use of veterinary medicines shall be in strict accordance with "Regulations on the Administration of Veterinary Medicines" and "Measures on the Administration of Veterinary Biology Products" and other documents.
8. Treatment of used equipments
Animal body or used equipments shall be treated before discarded.

Duration of Handling:
1. Handling of application: The applicant shall submit the application form.
2. Checking of "Permit on Animal Epidemic Prevention": Checking will be carried out within five days after the receipt of the application. Applicants will be informed to submit other necessary documents. If the applicant fails to submit all necessary documents within one month, then he/she should start with the application once again. On-spot checking of location, venue and layout will be carried out within ten days after the receipt of all necessary documents. Result of the on-spot checking will be informed to the applicant within five days after the on-spot check. Qualified applicants will be issued an "Permit on Animal Epidemic Prevention". Reasons will be given to applicants who fail.
3. Checking of "Permit on Animal Clinic": On-spot checking on clinic venue, equipment and instrument, staff, various systems, decoration and waste treatment will be carried out within ten days after receiving the re-check application. Result of the on-spot checking will be informed to the applicant within ten days after the on-spot check. Qualified applicants will be issued an "Permit on Animal Clinic". Reasons will be given to applicants who fail.
