Handling Procedure:
The following documents shall be submitted according to the type of application (Application of new permit, application to renew permit and application to reissue permit):
A. Documents to be submitted for application of new permit):
a. Application Form for Geological Survey Qualification (Three copies of original)
b. Qualification of legal person or non legal person (one copy of original and copy)
c. Documents of legal person and major technical staff (one copy of original and copy)
d. Professional qualification of medium to high level technical staff, name list of all technical staff. (One ccopy version)
B. Documents to be submitted for application to renew permit:
g. Original and copy of the original permit.
C. Documents to be submitted for application to reissue permit:
f. Proof that the original permit is lost (one copy version)

Handling Procedure: Submit the application - submit all other relevant documents - handling of application - inspection- checking - issuance of permit (if the application is handled by municipal bureau) or submitted to Ministry of Land Resources - The applicant will be informed - Issuance of Permit - Overall Inspection - Keeping file and publication of decision
