Application procedure: 1. "Qualification Certificate of Appointed Producing Factory for the Protective Equipment for People Defense Facility", in six copies;
2. Copy of business license;
3. Copy of certificate of professional title and work history (signed in person) of the professional technician;
4. Copy of qualification inspection report of the trial product, issued by the technological supervision institution appointed by State People Defense Office. Report shall be made based on every product.
All documents submitted, including copies, shall be affixed with official seal. Original documents shall be available for verification.
Execution procedure: 
　　1. The enterprise shall submit the application;
　　2. The general service window shall accept the application;
　 3. Shanghai municipal People Defense Office shall examine the documents, investigate and evaluate the production, production management organization, technical management system and quality assurance system of the applicant enterprise and then give the written comment. For the qualified, it shall issue the "Qualification Certificate of Appointed Producing Factory for Protective Equipment for People Defense Facility" and hand over this certificate and relevant application documents to State People Defense Office;
　　4. The applicant shall obtain the solution at the general service window.
