| Application for Verification of "Shanghai Qualification Certificate of Design and Construction of Technical Safety Protection Project", Level One or Level Two

Application procedure: The engaged organization shall obtain
the "Application Form for Qualification Certificate of the Organization Engaged
in Technical Safety Protection Construction, Level One or Level Two"
(hereinafter referred to as "Application Form for Qualification Certificate")
from the Technical Safety Protection Office of Shanghai Public Security Bureau
at every quarter (the detailed time shall be notified otherwise) and fill in the
form and submit it and following documents to the Technical Safety Protection
Office of Shanghai Public Security Bureau (all documents shall be affixed with
the official seal):
(1) Copy of the duplicate copy of business
license;
(2) Original and duplicate copy of "Shanghai Qualification
Certificate of Design and Construction of Technical Safety Protection Projects"
(hereinafter referred to as "Qualification Certificate");
(3) Copy of
achievement report of technical training;
(4) Technical Safety
Protection project contract;
(5) Copy of "Shanghai Conclusion of
Acceptance upon Completion of Technical Safety Protection Project"
(6)
For the technical safety protection project contracted and accomplished at other
Chinese place, the applicant shall submit the copy of acceptance document issued
by the local authority for technical safety protection.

level one or level two qualification shall apply to the Technical Safety
Protection Office of Shanghai Public Security Bureau for verification at every
quarter.

LAYOUT-GRID-MODE: char; LINE-HEIGHT: 135%">¡¡¡¡ (1) For the Qualification
Certificate qualified in the verification or degraded and to be replaced by an
inferior Qualification Certificate, the Technical Safety Protection Office of
Shanghai Public Security Bureau shall stick the qualification mark to the
original and duplicate copies respectively and affix the "Verification Seal of
the Technical Safety Protection Office of Shanghai Public Security Bureau".
After that, it shall return the Qualification Certificate;

LINE-HEIGHT: 135%">¡¡¡¡ (2) For the enterprise unqualified to the verification,
the Technical Safety Protection Office of Shanghai Public Security Bureau shall
order a correction within a certain period, circulate a notice of criticism,
degrade the qualification level or withdraw the Qualification Certificate
according to the extent of disqualification or the achievement of the same
year;
¡¡¡¡ (3) Any delay of application for verification shall mean the
automatic abandonment of qualification, so the Technical Safety Protection
Office of Shanghai Public Security Bureau shall withdraw the Qualification
Certificate of such enterprise.

Time limit: For the application with the
application documents full and in conformity with relevant regulations£¬the
Technical Safety Protection Office of Shanghai Public Security Bureau shall
finish the examination within 2 working days and issue the Qualification
Certificate to the qualified, or shall notify and return the application
documents to the unqualified within 2 working days.
Applicant's
qualification: £¨1£© Strictly abide by the "Methods on Management of Technical
Safety Protection Products", "Methods of Shanghai on Management of Technical
Protection of Public Safety"£¬and other relevant regulations of the authority for
public security.
(2) The design and construction of technical safety
protection project shall be strictly in conformity with national standards,
industrial standards and local standards. The enterprise shall provide timely
and reliable technical security and after service to the normal operation of
technical safety protection facilities.
(3) Receive the supervision and
management of the authority for technical safety protection in a voluntary
way.
(4) Requirements on the achievement of same year:
¢Ù The total
sum of finished projects of the enterprise with the qualification certificate at
level one shall be at least 1 million yuan;
¢Ú The total sum of finished
projects of the enterprise with the qualification certificate at level one shall
be at least 600 thousand yuan;
(5) All enrolled technical and
managerial staff shall receive the technical training organized by the authority
for technical safety protection and have passed the examination. |
