Processing Authority: Shanghai Municipal Highway Projects Quality Supervision Station
Application Formalities:

The approval will be issued by Shanghai Municipal Urban Infrastructure Project Management Bureau

1. The Letter of application shall contain the following:
 
（1）Name and address of the applicant.
（2）Name of supervising authority.
（3） Name, age, education, major, qualification, working experience, certificates, letter of appointment of legal person.
（4） Name, age, education, major, qualification, working experience, certificates of supervising staff. Copy of their certificates. Name list of administrative staff.
（5） Ownership nature of enterprises and Articles of Association.
（6） Achievements of the enterprise and major staff on highway projects.
（7） Approval of the establishment of the supervising enterprise.
（8） List of technical equipment.
（9） Scope of supervising business.
（10） Registered capital.
（11） Organizational structure.
2. Application Form for the Establishment (Grading and Upgrading) of Highway and Water Transportation Projects Supervision Enterprises
 
Processing Procedures:
I. The applicant shall submit the "Qualification Application Form", "Application Form for the Establishment (Grading and Upgrading) of Highway and Water Transportation Projects Supervision Enterprises" and all other relevant documents to Shanghai Municipal Highway Projects Quality Supervision Station.
II. Within twenty working days after the receipt of application forms and other relevant documents, the Station will make its decision of "Approve", "Not Approve" and "Need to Revise" in written form and inform the decision to the applicant.
III. The approval of Grade A and Grade B qualification will be approved and submitted to the Ministry of Transportation.
