Application procedure: the applicant shall submit following documents:
(I) Application for operation;
(II) Copy of certificate of approval of institution;
(III) Copy of qualification certificate of fire safety;
(IV) License of hygienic food;
(V) Copy of qualification certificate of principal and major managerial staff;
(VI) Copy of professional certificate of nursing staff;
(VII) Copy of medical service contract signed with the local first-grade medical institution or approval document of autonomous internal medical department;
(VIII) Standard of service and internal management system;
(IX) Certification of fluid capital, at least 50 thousand yuan for the institution with at most 150 beds and at least 100 thousand yuan for that with more than 150 beds;
(X) "Application Form for Operation of Social Welfare Institution in Shanghai".
Execution procedure:
(I) Acceptance: the application submitted to the local district/county Civil Affairs Bureau in charge of the old-people nursing institution to be established shall be accepted.
(II) Examination: the authority shall check the trueness, validity and legality of the documents, certificates and forms and conduct a field investigation.
(III) Verification: the authority shall notify the applicant of the result in written form.
After obtaining the license of operation, the applicant shall get its legal person identity registered at the local registration authority or the authority for civil affairs and the authority for industry and commerce in charge of the place of operation.

Time limit: (I) The application for the institution with 50-150 beds or the investment amount at or less than 10 million yuan shall be examined and approved by district/county Civil Affairs Bureau within 30 working days since accepting the application.
(II) The application for the institution with over 150 beds or the investment amount more than 10 million yuan shall be taken into the preliminary examination by district/county Civil Affairs Bureau within 15 working days since accepting the application, and, if approved, be submitted to municipal Civil Affairs Bureau for examination and approval, which shall be carried out within 15 working days since acceptance of the application.
Applicant's qualification: the old-people nursing institution with its application for establishment approved by municipal or district/county Civil Affairs Bureau and ready for operation.
