Execution authority: Application procedure: 1. Written application;
2. Report of site selection and construction layout plan; 3. Feasibility
report; 4. Credit certification of the applicant, either organization or
individual, for the establishment of medical institution; 5. Certification
documents of the basic information of the applicant, either organization or
individual, for establishing the medical institution; 6. Effective contract
or agreement when the application is made by two or more legal persons or
individuals jointly; 7. Other documents required by the authority for public
hygiene.
Note: If the application for establishment and for registration of operation
is submitted to the municipal authority for public hygiene, the applicant shall
submit the written comment on the preliminary review issued by the
district/county authority for public hygiene in charge of the medical

Time limit: Applicant's qualification: (1) The municipal authority for
public hygiene shall accept the applications for following medical institutions:
1. Third-grade hospital, second-grade hospital, and convalescent home; 2.
Sanatoriums, mother and children health hospital (station), disease control
hospital (station); 3. First aid center (station); 4. Clinical examination
center;
5. The medical institution run by non-government entity with 100 or more
beds; 6. Other medical institutions as required by the municipal authority
for public hygiene. (2) The district/county authority for hygiene shall
accept the applications for following medical institutions 1. First-grade
hospital;
2. Ambulant clinic; 3. Clinics 4. Nursing hospital (station); 5.
Health center (station, home), healthcare center; 6. The medical institution
run by non-government entity with less than 100 beds;
