Application procedure: 1. Application issued by the district/county Hygiene Bureau in charge of the medical institution. The application shall include the comment of preliminary examination and the comment on the conformity with regional hygienic planning and distribution of medical instutions1.
2. Application for establishment of medical institution
* If to establish the medical institution upon Sino-foreign joint investment or cooperation, the applicant shall submit the approval document issued by the local authority for public hygiene in charge of the medical institution.
3. Project proposal signed by the legal persons of both Chinese and foreign parties. The proposal shall include the name of project, type, location, ownership, total investment, registered capital, scale (beds or dentosurgical chairs), clinical department, term of business and proportions of interests, etc.

* The letter of intent of cooperation signed by both parties, if any.
4. Feasibility report.
5. Report of site selection.
6. Information of Chinese investor:
(1) Copy of registration certificate of organization or legal person, such as "License of Operation of Medical Institution" or business license.
(2) Copy of ID card of the legal representative.
(3) Original copy of credit certification issued by the bank.
7. Information of foreign investor (the applicant must provide the Chinese version that is affixed with the seal of foreign investor):
(1) Copy of registration certificate of legal person, such as license of operation of medical institution or business license.
(2) Copy of identity certification or passport of the legal representative.
(3) Original copy of credit certification issued by the bank.
8. Information that involves with the state-owned assets of the Chinese party

(1) With state-owned assets invested
House ownership certificate, construction layout plan, approval document of investment issued by the assets owner, report of assets assessment, verification document on the state-owned assets to be invested issued by State-owned Assets Management Office.
(2) Limited within lease of assets
House ownership certificate, construction layout plan, approval document of lease issued by the assets owner, lease contract or letter or intent.
