Application procedure: 1. Change of name of organization and legal representative: 

① Copy of "Business License of Legal Person of Enterprise" or "Business License" or "Registration Certificate of Legal Person of Governmental Organ";

② "Shanghai Registration Form of Change of Social Insurance";

③ "Shanghai Registration Form of (Change of) Organization that Implements Endowment Insurance", SEHN 0 Form.

2. Change of address of organization, postcode, liaison, contract phone and bank account;

① "Shanghai Registration Form of (Change of) Organization that Implements Endowment Insurance", SEHN 0 Form.

② "Shanghai Registration Form of Change of Social Insurance";

3. Change of ownership of organization

① Application;

② Approval document or legal person certificate of the ownership of organization before the change;

③ Approval document of the Compilation Commission of municipal and district Personnel Bureau

④ Copy of "Business License of Legal Person of Enterprise" or "Business License";

⑤ Copy of legal person code certificate;

⑥ Copy of approval document of change of ownership.

4. Change of termination of organization

① Application;

② Notification of deregistration issued by the authority for industry and commerce or the sentence of bankruptcy issued by the court;

③ Copy of approval document of merger issued by senior superintendent authority;

④ "Registration Certificate of Social Insurance".

Execution procedure:
　　1. For the organization that plans to change the basic information of social insurance, it shall apply to the execution authority for social insurance between 5th and 26th of every month and the execution authority shall settle the application within a day.
　　2. For the organization that plans to change the ownership, it shall fill in the "Shanghai Registration Form of (Change of) Organization that Implements Endowment Insurance", SEHN 0 Form and submit the application to the execution authority for social insurance before 26th day of the month when the approval is to be made. It shall be 10 working days from the acceptance to the settlement. For the application to be submitted to senior superintendent authority, the term of execution shall be prolonged accordingly but not longer than 1 month.

　　3. For the organization that plans to change the termination, it shall fill in the "Shanghai Registration Form of (Change of) Organization that Implements Endowment Insurance" and submit the application to the execution authority for social insurance between 5th and 26th of the month when the termination is to come into effect. It shall be 5 working days from the acceptance to the settlement. In case of any special condition under which the execution is to be prolonged, the term of execution shall not be longer than 1 month.
