(1) Written application for registration of change signed by the legal representative or the principal of the unit and affixed with the official seal of the unit;

(3) Registration form of changes, in three copies;
(4) Revised articles of association, in three copies;
(5) Verification form of articles of association, in three copies.
Execution procedure:

(III) Announce the change to public.
Standard and basis of charge: (I) Standard of charge
1. 40 yuan for every registration of changed item;
2. Fee of proclamation shall be charged as per the actual standard.
