district/county Civil Affairs Bureau or the administrative authority for social
welfare enterprise at the same level
Application procedure:
(I) "Qualification Certification of Shanghai
Social Welfare Enterprise";
(II) Approval document issued by the authority
for reform of economic system;
The municipal and district/county authorities
shall finish the examination of application documents respectively within 10
days since accepting the application when all the application documents are
complete.
(III) Valid assets evaluation report;
(IV) Information of constitution of
stock capita: valid certification of stockholders, either healthy employee or
handicapped employee, and respective sums;
(V) Copy of certification of
national support fund and relevant accounts of finance, managed by the authority
for civil affairs.

party and withdraw the national support fund as per relevant regulations to
allocate handicap-assistant stock capital and guarantee the implementation.
