If your passport is valid, you can make an application to add information with the Division of Entry-Exit Administration of the Shanghai Municipal Public Security Bureau. You must provide an explanation for your request and you must go to the office in person.
Procedure:
1. Present a completed Application Form for Changing Passport Information and three recent photos of yourself
2. Present your valid passport
3. Present related explanations:
- If you need a foreign name added to the passport information, you must present a visa notice, school admission letter and notarizations.
- If you need to add or cancel information about children in a passport, present a written explanation.
- If you need your birth date added to the passport, present your household registration, ID card or other material that explains your domicile.
