Application formalities: 1. An application form on authorizing cable TV equipment with network access;
2. Business license of the legal person;
3. Relevant documents on trademark registration;
4.Relevant documents on its meeting GB'T19000(ISO9000) standard;
5. Technical data of the products (enterprise standard, specifications of products and list of testing instruments, etc.);
6. Report on trial use from users (not for spare products);
For equipments whose trademarks are registered in China to apply for certificate on authorizing equipment with network access on ministry level, their certificates on equipment with network access on provincial level should be submitted.

Transaction procedures: transaction department is to produce an opinion on preliminary examination within 30 days from the day when the application is accepted. For those proved to be qualificed in the preliminary examination, they are to be notified to get prepared for appraisal on quality guarantee system and for product sampling inspection; for those not passing the preliminary examination, they are to be informed to supplement and correct application materials. Sampled products are to be sent to designated testing organs by applicant units, and testing organs are to produce a report on testing to the unit within 1 month. Transaction department is to do reexamination work based on the report on testing, and make a decision on whether to authorize or not within 30 days. Those getting authorized are to be issued a certificate on authorizing network access of equipment. Valid term of the certificate is two years.
     As for application for certificate on authorizing equipment with network access on ministry level, the testing job is to be done by testing organs examined, approved and authorized by State Administration of Radio, Film and Television, or be done by those examined, approved and authorized by administrative departments on technique supervision and also certified by State Administration of Radio, Film and Television.
