Such publishers should submit the "Registration Form for Enterprises That Publish Electronic Publications" and other relevant documents and licenses to the Shanghai Municipal Press and Publication Bureau, as follows:

(1) A letter of application, explicitly indicating the name, address and type of enterprise as well as name and address of its legal person and its major responsible person;

(2) the business license;

(3) the articles of association of the enterprise, and

(4) the professional qualifications and ID documents of the legal representative and the major responsible person.
