The following documents should be submitted by enterprises (including private enterprises):

(1) A letter of application, indicating the store name, address, the legal representative and the name and address of major responsible person;

(2) evidential documents proving the use of the office venue (e.g. a lease contract or the property ownership certificate of the party leasing out the property);

(3) the floor plan of the store;

(4) the name of store (three names should be submitted), and

(5) the ID certificate or qualifications of the applicant.
