How should people, who are subject to free-of-charge services for basic family planning, apply for reimbursement?

The applicant should fill in the application form Verification Form for Expense Reimbursement of Technical Services for Basic Family Planning and provide the original copy of related evidential documents at the request of the regulatory body when he/she applies for reimbursement as follows:

1. If he/she is married and a registered permanent resident of Shanghai, he/she should provide the household register, the marriage certificate, and the exact receipt of the clinic/emergent treatment (hospitalization) expenditure and the list of charges provided by the service institution selected by the district/county government or town/village government in charge of their registered permanent residence record.

2. If he/she is married and a registered permanent resident of another province or municipality, but has participated in the Shanghai Comprehensive Insurance Program for Immigrant Employees, he/she should provide the ID card, the marriage certificate, the Marriage and Child-bearing Certificate of Immigrant Employees, the Shanghai Temporary Residence Permit of Immigrant Population, the evidence of participating in the Shanghai Comprehensive Insurance Program for Immigrant Employees, and the exact receipt of the clinic/emergent treatment (hospitalization) expenditure and the list of charges provided by the service institution selected by the district/county government or town/village government of where he/she lives at present.

3. If he/she is married and has a Shanghai residence certificate, he/she should provide the ID card (not applicable to cross-national talents with Shanghai residence certificate V.B.), the Shanghai Residence Certificate, the marriage certificate, the Marriage and Child-bearing Certificate of Immigrant Employees (not applicable to cross-national talents with Shanghai residence certificate V.B.), and the exact receipt of the clinic /emergent treatment (hospitalization) expenditure and the list of charges provided by the service institution selected by the district/county government or town/village government of where he/she lives at present.

4. If he/she is a registered permanent resident of another province or municipality, and has married someone who is a registered permanent resident of Shanghai, he/she should provide the ID card, the marriage certificate, the Marriage and Child-Bearing Certificate of Immigrant Employees, the Shanghai Temporary Residence Certificate of Immigrant Population, and the exact receipt of the clinic /emergent treatment (hospitalization) expenditure and the list of charges provided by the service institution selected by the district/county government or town/village government of where he/she lives at present.

If the applicant entrusts another person to handle the reimbursement procedure, the trustee should additionally provide an original copy of his/her own ID card.
