What are the rules for keeping and using the Medical Services Record?

1. The owner should keep his/her own Medical Services Record booklet accurately and in good condition. If the Medical Services Record is altered or is missing any pages, it can no longer be used and the owner should apply for a new one at the Medical Insurance Center.

2. The Medical Services Record cannot be used by anyone other than the owner. Otherwise, the insurance participant who owns it shall be responsible for the incurred loss of Medical Insurance Funds and any other consequences.

3. When a medical insurance administration carries out an investigation on the related medical expenses, the insurance participant should assist them as necessary and show their Medical Services Record promptly.
