Instructions for Filling

1. The application materials shall be sequenced in the order as set out in the List of Application Materials in this Guide;

2. The documents prepared by the Applicant are printed with A4 papers, and those issued by the government and other authorities shall be provided in the size(s) as the original(s);

3. Copies of the application materials shall be clear and legible.

Formal Standards

I. Rules for entrusted applications. A motor vehicle driver can entrust an agent to change and replace the driving license, submit proofs of physical conditions, postpone the handling, and cancel the application on his or her behalf. When applying for the driving permit, the agent should submit his or her identification certificate and the application form jointly signed by the principal and the agent or the proofs of physical conditions.
II. Filling in the correct contact information is required. The applicant should fill in the real and correct contact information, including the mobile phone number and address (in the form of Room XX, No. XX, Lane XX, Road XX, District XX, Shanghai Municipality). The driver should be informed of the effective period of his or her driving license, the date of physical examination and other matters and go through the formalities as required. The traffic management department of the public security authority will inform the driver of the license-related matters by mail or SMS according to the contact information provided by the applicant.

IV. License photo requirements
1. The photo should be taken within 6 months before the application for a temporary Chinese driving permit;
2. The photo should be colored, squared, frontal view, bareheaded, half-length, no scarf, no uniform, and no colored spectacles; for those having long hair, their ears should not be covered; there should be no heavy makeups or jewelry on the head and face that may affect the fail recognition; the head and face should not tilt or turn to a certain angle, so as to ensure facial recognition.
3. White background, clear image, natural expression and posture without distinct distortion;

4. Photo size 32mm×22mm, head width 14mm～16mm and head length 19mm～22mm; 5. Electronic photo requirements:
(1) 50KB< size <1MB, jpg format, and resolution &ge;300dpi;
(2) electronic scan version or copy is not accepted;
(3) electronic information of the photo should meet the criteria.
V. Online handling of driving license-related matters. The applicant can apply for the handling of driving license-related matters on 122.gov.cn. For specific matters and procedures, you can visit http://sh.122.gov.cn/ to make an inquiry and application.
VI. Effective period of proofs of physical conditions. Proofs of physical conditions should be effective for 6 months from the issuance date.
VII. Effective period of driving licenses. Driving licenses can have an effective period of 6 years, ten years and long-term, which is indicated on the driving license.
