[Time of acceptance]

Foreigners could apply for physical examination upon entry during 08:00-11:00 a.m. from Monday to Friday. Besides, we are to arrange such physical examination for foreigners in the afternoon from Monday to Thursday and on Saturday based on change in the number of such foreigners, without fixed schedule. The specific arrangement is subject to the appointment of the very day.

[Physical examination appointment]

The health assessment and physical examination for foreigners is subject to appointment, so please appoint two weeks in advance. There are two ways of appointment:

1. Appointment online: Appointmenton this website.

2. The applicant or proxy of applicant go to the No.2 window of the Public Service Center with the identity document or duplicate of the applicant (passport/Exit-Entry Permit for Travelling to and from Hong Kong and Macao/Mainland Travel Permit for Taiwan Residents/travel certificate) for appointing. The on-site appointment is accepted during 8:00-11:00 from Monday to Friday and and during 13:00-14:00 from Monday to Thursday (excluding national holidays).

2.Fill in theInformed Consent Form.

3.Identity documents:

(1)  Foreigners:  Passport, 1 original+1 duplicate (photo page +visa page/visa-free entry page + information change page).

(2)Residents of Hongkong and Macao: Permit or travel certificate of Residents of Hongkong and Macao to and from Chinese Mainland, 1 original+1 duplicate (photo page +visa page/visa-free entry page + information change page).

(3) Residents of Taiwan: Permit or travel certificate of Residents of Taiwan to and from Chinese Mainland, 1 original+1 duplicate (photo page +visa page/visa-free entry page + information change page).

(4)  Chinese citizens having overseas permanent residence: Passport, 1original+duplicate1 (photo page +visa page/visa-free entry page + information change page).

4.Three identical 3.5*5.3cm bareheaded photochromes taken within the last six months.

5. Relevant certificates and documents:

(1)  For staying due to employment: Certificates of employment issued by the administration of foreign experts affairs in relevant place, business license (duplicate), employment permit of the applicant (original and duplicate)

(2)  For staying due to study in China: Student ID card or admission letter (original and duplicate).

(3)  For staying of accompanying families: Duplicate of the business license of the employer of such accompanying families and other relevant proofs.

(4) For staying due to visiting families, purchasing housing, healing, getting married, settling down in China and other reasons: The Registration Form for Temporary Residence (duplicate) issued by the public security organ or property ownership certificate (original and duplicate) of the applicant and other proofs concerned.

6.If carrying the health examination record issued in China and overseas within the last six months, please show the original health examination record issued by the hospital designated by the embassy of the country (or region) in which such examination is received for reference, including the original of the lab test report and X-ray film.

[Notes]

1. Please go through the procedures of health assessment and physical examination as soon as possible after entering the country, so as not to affect the subsequent visa application.

2.  Keep an empty stomach in the five hours before the physical examination. Drinking water is allowed.

3.Take some cash.

[Certificate taking]

Foreigners receiving physical examination could take the certificate in person or via courier service.

1. In the case of the foreigners receiving physical examination taking the certificate in person, they should go to the No.2 window of the Public Service Center with the receipt and original identity certificate within four working days after the examination.

2.In the case of courier service used, foreigners receiving physical examination should entrust the courier service in person after the examination, and the physical examination certificate would be send to them within four working days after such examination.

To ensure the security and privacy of the examination result, if the foreigners receiving physical examination want to change the way of taking certificate after it is determined, they should go to the No.2 window of the Public Service Center in person.

[Re-examination for abnormalities]

In the case of needing re-examination for abnormalities found during the health assessment, this center is to timely contact the foreigner receiving physical examination and sent him or her the notice of re-examination. Such foreigner should go to the designated unit for re-examination within the specified time with his or her identify document (original) and the voucher of payment of physical examination fee.
