(2) Application procedures

When applying for filing at the service window, digital application documents shall be submitted in the form of CDs. Procedures of online application:

1. Visit “Legal person” session on the Government Online-Offline Shanghai platform - Visit “Filing for social service providers on environment monitoring” in the “Other categories” session under the Shanghai Municipal Ecology and Environment Bureau - Click “Apply immediately”.

2. Click “Legal person Ukey login” to register an account. Before registration, please make sure that the Ukey device of legal person single certification is inserted into the computer (Note: Both local organizations or ones of other provinces and cities shall undergo in-person application for the Ukey device of legal person single certification at designated service windows. Login or registration process requires an assisting software and is recommended to be conducted using browsers including IE11, 360 and Chrome).

3. Upon registration, complete basic information and documents for filing (Registration Form of filing for social service providers on environment monitoring, Self-declaration document signed by the legal person or people authorized by the legal person, and the Certification on Qualification of Examination and Testing Institutions) shall be submitted into the system.

(3) Instructions on information submission

1. All application documents shall be placed in accordance with the sequence specified in this instruction book;

2. Digital version of application documents shall be clear;

3. The Certification on Qualification of Examination and Testing Institutions shall be valid at the time of submission and shall be updated in a timely manner;

4. As documents required for information change are the same as that for new registration, social service providers on environment monitoring that apply for information change are only required to submit the changing information and relevant supporting documents.
